What is endodontic treatment, or so-called “root canal”?

Endodontic treatment, or root canal treatment, is performed when there is a serious cavity in a tooth, including the pulp. This is most often a consequence of untreated decay, but also the result of trauma. When the pulp is infected there is also pain, as it is highly innervated. To save a tooth, the source of the infection must be eliminated. u003Cbru003Eu003Cbru003EThe diseased or dead pulp has to be carefully removed, and then the inside of the canals has to be cleaned of bacteria, with the final goal of filling the canals tightly and rebuilding the damaged part of the crown. In this way, we eliminate the source of the disease and save the tooth from destruction. The most important thing in root canal treatment is to find all the canals and remove all the pulp – up to the root tips. Otherwise, the inflammation will continue to develop and the patient will be at risk of repeated root canal treatment. Microscope root canal treatment step by step

1 It starts with accurate diagnostics. At STOMO Clinic, we believe that dental CT scans best support effective endodontics, and especially accurately depict all the canals of the treated tooth (remember, teeth can have more canals than the anatomy assumes). On a standard X-ray it is not always possible to find all the extra canals and accurately depict their shape. Thanks to CT scan diagnostics, we can also precisely plan treatment. At our clinic, we use a state-of-the-art Carestream Dental 9600 cone tomograph. u003Cbru003Eu003Cbru003E2 We then administer anesthesia to the patient to make the entire procedure 100% painless.u003Cbru003Eu003Cbru003E(3) At this stage, we clean the tooth of decay and open the tooth chamber. Under the microscope, we thoroughly clean the canals of diseased or dead pulp and remove bacterial toxins. Why shouldnu0027t you wait for root canal treatment for a tooth?

Inflammation of the pulp is most often a consequence of untreated caries, but can also be the result of poor treatment or mechanical trauma. At first, the sick tooth can, as patients say, chirp, gently aching, and over time it begins to react to cold and hot meals. In the acute version, pulpitis means fever and severe pain. Do not delay root canal treatment, because pulpitis can cause inflammation in the jawbone or jawbone, and consequently bone loss, lead to the formation of a cyst, not to mention, of course, the loss of the tooth. u003Cbru003Eu003Cbru003EAnd one more thing, an infected pulp is a source of infection that can harm the whole body. There is a whole range of focal diseases, the source of which can be the tooth itself. These include, for example, inflammation of the kidneys, meninges or endocardium of the heart.

Removal of broken tools

A broken tool stuck in one of the canals of an endodontically treated tooth is a topic that cannot be trivialized. At STOMO Clinic, we deal with the removal of tools left in tooth canals. Such problems are reported to us by patients from other clinics in Wroclaw. They need to be resolved because a broken tool prevents the tooth from being properly cleaned of pulp. It is also potentially a source of infection, which can even lead to bone inflammation. Fragments of endodontic tools are removed under a microscope, due to the fact that they are microscopic in size and invisible to the naked eye. Most often, the tools become detached in the canal due to its complex, curved structure, as well as due to, for example, canal fusion. Thus, this is not a malpractice, but a complication, arising during the procedure. u003Cbru003Eu003Cbru003EAt STOMO Clinic, we have the comfort of being able to precisely locate the smallest tool debris using a cone tomograph and a dental microscope. Does root canal treatment under the microscope hurt?

Root canal treatment at STOMO Clinic in Wroclaw is a completely painless procedure. Precise treatment is first performed under a microscope, usually in a single visit, and the patient is well anesthetized with the best grade of anesthetic substances. The procedure involves a thorough diagnosis of the condition of the tooth, imaging the canals, and then cleaning, disinfecting and filling them. Today, during root canal therapy, the tooth is no longer poisoned, that is, the nerve running inside it is not killed. Such methods in the past caused pain, with the patient repeatedly reporting to the dentist, waiting for the tooth to “give up” and be dead. Hence the whole stereotype about the painfulness of root canal, originating from times that have thankfully passed into history. u003Cbru003Eu003Cbru003EModern endodontic techniques are based on completely cleaning the canals of pulp and bacterial toxins. As a result, the tooth no longer has innervation, conducting pain impulses, and the patient does not have to endure pain while waiting for the tissues to die. They are simply “removed” from the tooth. u003Cbru003E

What to do if a tooth still hurts after root canal treatment?

After root canal treatment, the tooth may still hurt for a few days. As a rule, post-operative pain subsides after 2-3 days. During this time, take prescribed painkillers or antibiotics, and try not to strain the tooth while eating. Most importantly, post-operative discomforts should diminish day by day until they disappear completely. If they worsen, persist longer and there is no response to antibiotic treatment, see your doctor. He or she will perform a diagnostic test, as the pain may be a consequence of the fact that not all the dental canals were treated during the procedure or the canals were not cleaned to the tops. u003Cbru003Eu003Cbru003EAt STOMO Clinic, we work with a CT scanner, which allows us to find all the canals of endodontically treated teeth (teeth can have more canals than anatomy textbooks describe). We then clean all found canals of infected pulp and fill them solidly up to the very tops. The effectiveness of the “root canals” is always confirmed by the diagnostic radiographs we perform after treatment. In this way, we minimize the percentage of repeat root canal treatment. u003Cbru003Eu003Cbru003EThe pain of a root canal-treated tooth can occur even after many years. It is most often the case that the canals were once RIGHTLY cleaned, but the remnants of the infected pulp caused another inflammation in the apical area over time. The inflammation can even be in a purulent form, in which case – even years after treatment – there is pain. But just as well, the inflammation may not make itself known, but cause bone atrophy and cyst formation. u003Cbru003Eu003Cbru003EThatu0027s why we sensitize patients to check with their dentist with the help of CT scans for teeth that were root canal treated years ago. There is another, not at all uncommon situation. Pain after a long time after the so-called root canal can also be given by a broken tool, a fragment of which remained in the canal and prevented its cleaning. It needs to be removed, otherwise another inflammation will occur. At STOMO Clinic, we also deal with the removal of broken tools from dental canals, which patients from other dental clinics come to us with. We first accurately diagnose the condition of the tooth with a top-of-the-line Carestream Dental CS 9600 CT scanner (the highest safety on the market), and then perform a repeat root canal treatment, or reendo.

Does a root canal treated tooth darken?

Itu0027s true that a root canal-treated tooth can darken. And since its appearance then stands out from the rest of the dentition, it is worthwhile to apply the appropriate aesthetic treatment. At STOMO Clinic, we perform teeth whitening treatments after root canal treatment. It is worth reassuring patients that this is not an unusual situation, discoloration of dead teeth happens commonly. u003Cbru003Eu003Cbru003EWhy does a dead tooth turn yellow, gray or even sino-brown? Most often because blood from an infected pulp enters the dentin tubules. Discoloration can also be a side effect of the chemicals by which the tooth was cured, and therefore saved. At the same time, however, in some patients a dead tooth may remain white for life, while in others it may darken over time or change color immediately. u003Cbru003Eu003Cbru003EWhat to do if we find ourselves in the latter or third group? To whiten a tooth that has darkened after root canal treatment, we use intracanal bleaching. After root canal treatment and root canal filling, we then apply a special bleaching agent to the inside of the tooth crown. We also secure it to prevent it from getting inside. The cement also isolates it from inside the mouth. The patient comes to STOMO Clinic every few days to replace the whitening agent and check the degree of whitening. In summary, the whole process takes about 2-3 weeks, after which we fill the bleached tooth with a composite filling. u003Cbru003Eu003Cbru003EHowever, if the tooth is too brittle, due to the preservation of too small a part of the crown (large carious cavities, extensive cracks), a better result is:u003Cbru003E- first strengthening the tooth with a crown-root inlay,u003Cbru003E- and then placing a crown, the color of which will perfectly harmonize with the aesthetics of the smile.u003Cbru003EThis is the best solution in terms of aesthetics, because the porcelain crown covers discoloration well. It is also beneficial for the long-term survival of a dead tooth in the mouth. u003Cbru003Eu003Cbru003E

How long will a root canal treated tooth last?

In the past, it was thought that a root canal-treated tooth was a short-lived solution. This was said because endodontic techniques were imperfect. However, it must be admitted – dentistry could not cope with the removal of pulp from very narrow and curved canals. Fortunately, now we have such modern methods that we can precisely clean and tightly fill even very narrow and very curved canals. u003Cbru003Eu003Cbru003ETo this day, we still have patients who require repeat root canal treatment because they were treated with old techniques. It turns out that they still have an inflammatory focus in the problem tooth, even though they treated it in the past in multiple visits. Meanwhile, at STOMO Clinic, root canal treatment under the microscope is usually done in a single visit. It involves a complete cleaning of the canals from the infected pulp, followed by a tight filling. u003Cbru003Eu003Cbru003EIt is also not a problem for us if a tooth has a severely damaged crown. We can first strengthen it with a root-canal post, and then rebuild it with a prosthetic crown. A properly treated, strengthened and rebuilt tooth will last in the mouth for many years, actually as long as natural dentition. However, it should be checked regularly, preferably every 6 months during a standard dental inspection.

Root canal treatment of teeth under the microscope – contraindications

Actually, there are no categorical obstacles to start primary root canal treatment. Also, age is not a contraindication, so we treat the teeth of both older patients and teenagers with root canals. However, if the root apices are not yet fully developed, we wait with “root canal” in younger patients for the completion of tooth development. On the other hand, the risk group for repeated root canal treatment are patients with certain types of congenital heart defects, a history of endocarditis or a heart valve (we qualify the patient in this regard during the consultation). Is a “wayward” tooth worth root canal treatment?

Implantology is developing at a rapid pace and you think to yourself, why do I need this broken tooth. Iu0027ll pull it out and put in an implant. This is not quite the right approach.
